Mastopexy or mammary lifting is a cosmetic surgery for breast lifting, one of the biggest challenges of the plastic surgeon, with the purpose of repositioning the breast on the thoracic wall, based on aesthetic criteria.

Amongst the causes of breast ptosis we enlist: pregnancy and breast feeding, aging, skin and tissue elasticity reduction. The Cooper ligaments are responsible for maintaining the attachment of the mammary gland on the muscular fascia are. With aging, these lose their elasticity, leading to lowering or mammary ptosis. Its correction involves the lifting of the gland parenchyma, removing skin excess and repositioning the areola. There is a classification of the breast ptosis:

1st grade (light ptosis):
the nipple is situated a little bit under the inframammary groove, but above the inferior pole of the gland

2nd grade (moderate ptosis):
the nipple is situated under the inframammary groove, a little bit above the inferior pole of the gland

3rd grade (severe ptosis):
the nipple is situated under the inframammary groove and under the inferior pole of the gland

Pseudoptosis:
the gland is ptosed, but the nipple is situated at the level of the inframammary groove

The intervention is made under general anesthesia or under local anesthesia with intravenous sedation, in case there is a small tegument excess. There are more ways in realizing the mastopexy. One of them is made with three incisions: one around the areola, other gets down from the inferior edge of the areola until the inframammary groove, and the third is horizontal under the breast. After the skin excess is removed, the nipple and the areola will be moved onto a higher position. The areola is being shrinked, in case it’s too extended. The skin, initially situated above the areola will be removed for reshaping. In some situations, the horizontal incision under the breast can be avoided. Otherwise, there can be avoided both the horizontal and the vertical ones, depending on the aspect of the breast. The results of the mastopexy are stable as long as there aren’t any important variations of the body’s weight or breast volume caused by a pregnancy, for example. In any case, the position of the areolas doesn’t radically changes after this surgical intervention, but only the position of the mammary gland can variate moving towards the inframammary groove.
